This questionnaire provides an opportunity for adult carers (18+) to share your experiences, views and opinions of the services available for carers in Herefordshire provided by Herefordshire Council and NHS Herefordshire.
We also welcome the views of young carers (under 18) and would invite you to provide any comments you have about your experiences as a carer, or of carers' services in Herefordshire, by contacting Amy Pitt on 01432 344344 or email apitt@herefordshire.gov.uk. All individual responses will remain confidential. Any information you provide will only be shared between a small number of researchers at Herefordshire Council and will only be used for the purposes described. Any comments you provide may appear in anonymised form in the published results. The key findings of the form will be circulated in the autumn.
Herefordshire Council and NHS Herefordshire are committed to ensuring that the services meet the needs of the individuals and your comments and suggestions will help us to improve the services available.

| How to obtain the full results | The results will be available online and people will be made aware by the cover letter sent with the questionnaires and on the introduction page at start of the questionnaire. Also a web link and email can be sent to appropriate users as well as further consultation events, Herefordshire Carers Support service and depending on budget paper advertisement. The strategy will be launched at the Carers Rights Day on 2nd December 2011 and a formal consultation on the document will follow. |
