The questionnaire aims to find out what it is like to live in Herefordshire as a disabled person. We want to know what is good about living in the county, and what can be improved.
The Council has adopted the Social Model of Disability. This means that we distinguish between impairment and disability, and recognise that social barriers are what disable people, not their impairment. These barriers include people's attitudes to disability, and physical and organisational barriers. The Council is therefore working towards identifying and removing as many of these barriers as possible.

| Description | This questionnaire is aimed at disabled people who live, work or study in Herefordshire, and those who have a long-term illness or health problem - some examples include diabetes, HIV, cancer and arthritis. A disabled person is defined in the Disability Discrimination Act as someone with a physical or mental impairment that has a substantial and long-term (more than 12 months) impact on their ability to carry out day-to-day activities. You might also want to complete this questionnaire on behalf of someone else. |
